Reefton Health Services Integration Project
The goal of this project is to re-design existing services to combine individual components into a single integrated model of care for the Reefton community in a sustainable and effective manner.
In particular, the integration of the primary health care practice with a neighbourhood nurse service and the development of both inpatient and community based services for older people and those who have long term health care needs.

WISE Older Persons Project
After a couple of years work and the help of many people involved in services for older people, in December 2006 the West Coast DHB released a plan for older people's services - the West Coast Improving Services for the Elderly Plan 2006-2016, known as the "WISE plan".
