This page introduces you to the Dementia Project.

Introduction

Dementia is the second pathway to be developed through the Patient Pathways Project.

Sue Wells will be working alongside health professionals and service providers to gather information from key staff involved in the pathway to establish best practice guidelines and service requirements.  Interviews with staff commenced in January 2009.

After all information has been gathered and process documents created, the information will be collated into a single file for the journey in its entirety and released for consultation by all stakeholders.
