Integrated Stroke Service
Integrated Stroke Service is the first pathway to be developed through the Patient Pathways Project and follows on from work begun as part of a patient journey mapping exercise undertaken earlier this year.
Alison McDougall and Sue Wells will be working alongside health professionals and service providers to develop service provision framework around the stroke service in line with suggested improvements from patient journey mapping work already completed.
In order to obtain the information required for the new pathway, working groups will be formed to establish best practice guidelines and service requirements. A meeting with each working group will be scheduled to document the information. After all groups have met the information will be collated into a single file for the journey in its entirety and released for consultation by all groups before agreement and sign off is obtained through the Patient Pathways Steering Group.
