Continuity of care

The elective major joint replacement pathway has experienced difficulty with continuity of care for some patients as a result of reliance on locum surgeons, the reduced level of staff in Allied Health, and the need for improved discharge planning processes.

 

Goal

The main goal of this exercise is to consider how these issues can be managed in a safe and efficient way and to develop new, or alter existing, processes to streamline the patient journey. A map of the current pathway will provide us with a means of testing any proposed changes so that we can view the impact on the total journey before implementation.

A patient consultation group will also be formed to provide us with feedback on their experience of the journey.
