This page outlines the procedure to follow when wanting to suggest a pathway for review.

The Patient Journey Scoping Tool

If you are aware of a pathway that could benefit from a patient journey improvement exercise please complete the Patient Journey Scoping Tool and then submit it to the Patient Journey Steering Group.

The purpose of the form is to identify the potential depth and scope of an exercise, assess which areas of the pathway cause the most difficulty and provide a means of judging how urgent the issues are.  It is important the Patient Journey Steering Group has as much information as possible to understand the nature of the pathway and reasons a review is required.
