This project to be undertaken over the next six months was established as a result of prior work and consultation with the Reefton community. The project is funded from the Rural Innovation Fund administered by the Ministry of Health and is an exciting opportunity to improve current services for Reefton.

Project Goal
The goal of this project is to re-design existing services to combine individual components into a single integrated model of care for the Reefton community in a sustainable and effective manner. In particular, the integration of the primary health care practice with a neighbourhood nurse service and the development of both inpatient and community based services for older people and those who have long term health care needs.
The project will result in:
- Further development and implementation of an integrated service model to include:
- A continuum of health services in Reefton including home support services, 10 aged care beds and 5 short stay beds.
- Access to advanced health services provided outside of Reefton
- Utilisation of information technology to enhance integration and information sharing
- Consideration of facility requirements for an integrated approach

One organisational structure for Reefton Health Services including:
- Develop a 24/7 roster to cover all service requirements
- Additional staff appointments working across the Reefton community (FTE allocations to be confirmed):
- Health Services Manager
- Home Support Services Coordinator
- Allied health staff

Re-design and development of Home Support Services that considers:
- Delivery or co-delivery of services by a provider external to West Coast DHB
- Restorative principles and approach
- Establishment of a plan for 5-10 supportive housing units

A workforce learning and development plan that addresses:
- Learning and development needs associated with integration of services and service development
The Project Includes
- Examination of roles, scopes of practice and opportunities to align roles within an integrated service model
- Role development of new and enhanced roles
- Developing recommendations and action plans
- Recommendations and/or policy development for IT and information sharing
- Recommendations to WCDHB Corporate Services Manager regarding minimum facility requirements
- Development of processes and/or procedures to support implementation and new ways of working
- Consideration of the impact on the workplace and training
- Service description / specification development

The Reefton Health Services Integration Project is progressing smoothly. The first stage of the project has been to form a working group, gather information from staff and identify strengths of current services including examples of integration where this already occurs.
Work is currently underway working with staff to outline a preferred organisational structure that will support integration of services for Reefton. Once the preferred organisational structure has been determined, this will be shared with staff and the wider community. This is likely to occur in early April
