Carelink Launched

The West Coast District Health Board launched its Carelink service in Greymouth yesterday.  Carelink provides Needs Assessment and Service Co-ordination (NASC) for people who have an identified need for long term support services.

Carelink will provide a central coordinating agency which will help those who live with some disability in the West Coast community to receive the right support at the right time.

Carelink will streamline services by using dedicated staff which will enable a more expert and timely service to elderly persons and those with disabilities that require support in the community, rest home and hospital settings.

People can self-refer to Carelink or be referred from a health professional or community agency. The service is based in the "Brick Building" opposite the Greymouth railway station

The Carelink team includes Assessment, Treatment and Rehabilitation and Dementia Outreach Clinical Nurse Specialists, Needs Assessors and a Service Coordinator and will be based in Greymouth, but include staff in Reefton and Westport.
