All personal information collected by the West Coast District Health Board will be managed in accordance with the requirements of the Privacy Act and Health Information Privacy Code.
None of the personal information collected will be disclosed to a third party without the written permission of the individual concerned, except in instances where the disclosure of the information is a legal requirement.
The main reason for the collection of personal health information is to enable care and treatment to be provided to you. You do not have to provide any information unless you are required to do so by a particular law. If so, you will be informed that the law requires you to provide the information and what the law is.
You have a right to request access to the information that relates to you that is held by the West Coast District Health Board.
If you believe that your rights under the Health Information Privacy Code have been breached you can make a complaint either to the West Coast District Health Board or to the Privacy Commissioner (P.O. Box 466, Auckland).
